Call your insurance provider or visit your online member account to review your coverage, benefits, and eligibility. Steps 2-4 reflect ALL services included in chiropractic care at our clinic.

If you are a new chiropractic or acupuncture patient, ask if the following CPT codes are covered: 99202, 99203, 99204. If you are an established patient with a new injury: 99212, 99213, and 99214. Ask if it applies to your deductible.

You’ll want to look up the following CPT codes: 98940, 98941, 98942, and 98943. Verify how many visits are covered per calendar year and whether the benefit is subject to a copay, coinsurance, or deductible.

Our chiropractors frequently bill CPT codes 97110, 97112, 97535, and 97140 in addition to the codes listed in Step 3. Verify that these services are covered when performed by a chiropractor, whether they are subject to an additional copay, coinsurance, or deductible, and whether billing them causes the visit to be processed as a rehabilitative/physical therapy visit, in addition to a chiropractic visit.

Verify coverage for CPT code 97124 and specify that the service will be provided by a Licensed Massage Therapist (LMT). Please note that our office does not bill as a facility. Ask whether the service is covered, whether it is subject to the patient’s deductible, copay, or coinsurance, and whether any visit limits or preauthorization requirements apply.

Ask about CPT codes 97810, 97811, 97813, 97814, as well as 97139, 97124, 97026, and 97140 when performed by an acupuncturist, as coverage may vary by provider type. Confirm whether these codes are subject to the deductible and whether billing them will cause the visit to be processed as a physical therapy visit.
